Physicals Plus is a comprehensive wellness clinic, owned and operated by a Board Certified Family Nurse Practitioner who has Full Practice Authority in the State of Illinois. While we offer works, schools, sports and CLD (DOT) physical exams we also provide a comprehensive list of services including primary care, home-based primary care, weight management, chronic disease management, vitamin injections, Skin Pen microneedling, teeth whitening, chemical peels & Tox treatment (Xeomin or Botox). How do I book an appointment? You can book an appointment online 24 hours per day at on our website

Do you accept insurance? Yes we accept the following insurances: Blue Cross Blue Shield (PPO, MMAI, Medicare Advantage), AETNA (PPO, Better Health, Medicare Advantage), Molina (MMAI, Medicare Advantage, PPO), Ambetter, Cigna (PPO). But please verify your coverage with your carrier. You are also responsible for any deductibles or copays.​

Can I pay cash? Yes, if you don't have insurance or have and an insurance we don't take we have a self-pay discount option.

How does your weight management program work? We can see you for weight management for a one time visit or monthly. We recommend at least a 3 months commitment. We can bill your insurance but you are responsible for any copays or deductibles. We also have a self-pay option available for monthly and 3 month options. More information can be found here
